Reablement Services Update

Reablement services are early intervention and prevention services that assist people to stay independent for as long as possible by providing a little more help at an early stage. Reablement services include things like: support to recover from the effects of illness; help to manage a long term condition from someone with experience of a similar condition; help to safely maintain home and garden; training to get a job or return to work after a break; support to start taking some exercise. Not only do these services make sure people can stay in their own homes for as long as they want, they also help keep down the costs of care in the future.
This meeting will be a chance to hear from the providers involved with the 5 new reablement Extended Research Projects in B&NES that will be delivering a range of services locally. These projects are providing integrated health and social care support, intensive home from hospital support, handyperson and minor adaptations services, step down accommodation and support, telehealth to support congestive heart failure. The services are being provided by Sirona Care and Health, Way Ahead, Age UK B&NES, Care and Repair and Somer community housing trust. There will be plenty of opportunity to ask questions and discuss the new services. There will also be a chance for groups at the meeting to network and promote their services and explore how their services might relate to the new reablement projects.
