NHS South Gloucestershire is reviewing Community Health services in the area in preparation for awarding a new contract from April 2013. At the moment North Bristol Trust has the contract. Health services based in the community provide care for patients in their own home and other places within the community. This includes community nursing; health visiting; continence services; outpatient podiatry; a range of community therapy services and specialist services. NHS South Glos wants your views on current and future community health services and these will be used to support the future design of services.
