The Care Forum helps to make health, care and the voluntary sector work better together. Part of this is by signposting people to services via our Well Aware community directory.
This work is funded by Bristol City Council, South Gloucestershire Council, Bristol North Somerset South Gloucestershire Clinical Commissioning Group.
We are really pleased to announce a continuation and development of this relationship. As of 2020 we will continue to make sure people, including carers, can find good quality, hyper-local information about health and wellbeing services via our directory of services – Well Aware. This will be as the chosen provider for a new model of collecting, tagging and assuring information that Bristol City Council has developed with our input.
We will be creating Bristol City Council’s public facing website and delivering the quality assurance for the service information in South Gloucestershire and Bristol.
