Invitation to help plan Community Services

Community health services provide care in or close to people’s homes to help them feel supported, avoid having to go to hospital or help them when they come out of hospital. These services work closely with general practices. Examples include community nursing, physiotherapy and speech and language therapy, among many others.

NHS Bristol, North Somerset and South Gloucestershire CCG is the organisation that plans and pays for health services. Contracts for community services are coming to an end over the next few years so we are planning how to offer consistent community services across the whole region. Our focus is on having a service that meets people’s needs both now and in the future, putting patients and carers at the centre of care.

We want to hear what the ideal community services look like to you. We are running four workshops to help us plan. We are inviting partner organisations, clinicians, people who use services and their families and others to tell us what you think.

Please respond to this email to let us know if you are interested in attending a workshop on one or more of these topics:

18th October 2018, 9.30-12.30 at Weston Campus (focus on community services teams working in a more joined up way)

1st November 2018, 9.30-12.30 at a central Bristol venue to be advised (focus on intermediate care)

1st November 2018, 1.30-4.30 at a central Bristol venue to be advised (focus on specialist support and advice, particularly nursing)

15th November 2018, 9.30-12.30 at The Vassall Centre, Gill Ave, Bristol BS16 2QQ (focus on locality hubs – buildings where a range of community services could be offered)

We will start each session with an overview of the process we are using and the overall vision. We will then do group work to hear more about the specific workshop topic. This is your chance to have your say on the way services could work, so please bring your knowledge and experience and share this with us.
