Healthwatch South Glos End of life care survey

At Healthwatch, we believe that local people should always have a chance to have their say about local services. This survey will be used to help your local councillors to understand your views when they think about how end of life services work across South Gloucestershire. Your elected representatives will be presented with your thoughts before a Health Overview and Scrutiny Meeting in November 2016.
