Somerset: Do you have an Education Health and Care plan or have you been involved in one?

Somerset County Council (SCC) need your help to find out how Education, Health and Care Plans (EHCP) are working for
children, young people and families in Somerset. SCC are working with In Control using POET
(Personal Outcomes Evaluation Tool) questionnaires.

There are three different questionnaires, so please select the one most appropriate for you:

1. Young People
2. Parent Carers
3. Practitioners (workers)
