The Care Forum launches it’s South Gloucestershire Community Winter NHS Survey

The Care Forum’s Dialogue Team have launched a new survey to find out how people living in South Gloucestershire are experiencing NHS Services in their local area.

The Care Forum and Dialogue are passionate about placing your thoughts and experiences at the center of our thinking as we help shape health and care, engagement and other services across South Gloucestershire.

The NHS is under increased pressure impacting many people in South Gloucestershire and the country. We want to know how these NHS services have been for you by listening to your stories.

Help us build a picture of what NHS services look like for people living in South Gloucestershire.

If you leave your contact details, you will automatically be entered into a prize draw for a chance to receive a £10 gift card!

The feedback and stories you tell us will be fed back to local South Gloucestershire leaders and help us to enact positive change.

We have an created an online survey which we are encouraging people from South Gloucestershire to take part in and tell us their story. We will also be working with local groups and organisations across South Gloucestershire to get the people that they work with to share their stories.
